    Personal Data Incorrect in Official University Records

    Problem

    A SU client (student, staff, faculty) reports to Service Center consultants that they believe their name is spelled incorrectly or there is some other problem (i.e., their SSN, address, etc. is incorrect). They wish to have it changed.

    Solution

    Information Technology and Services does not own official University records. That data is owned by Central Records and/or Human Resources. We cannot change a client's demographic or personal information on official University records.

    If a client is requesting a username change, they must complete this step before Service Center consultants can process the request. ([suid:Procedure for NetID Username Changes). To have information corrected in University records, do the following:

    Students

    Contact the Office of the Registrar registrar@syr.edu or visit their website or call 443-2422 to obtain information on how to proceed with getting the data corrected.

    Employee Demographic Change

    If you experience a change in your legal name or sex designation, follow the steps outlined below to submit an Employee Demographic Change form to HR Shared Services.


    The Employee Demographic Change Form is an eForm.  Our eForms are secure, fast, and accessible.



    To access the form in your MySlice portal, follow these easy steps:

    1. Log into your MySlice portal.

    2. Under “Employee Home” select the “Employee Resources” tile.

    3. Within the “Employee Resources” tile select the “eForms” tile.

    4. Expand the “Human Resources” section by clicking on the grey triangle on the left side of the page.

    5. Click on the “Employee Demographic Change Form” link to be taken to the form.

    6. Complete the eForm by answering all the questions and securely uploading any required documentation.

    7. Ensure that you click the “Submit” button at the bottom of the eForm to send your completed application to HR Shared Services for review.

    8. Communication will be sent to your Syracuse University email regarding the status of your form.
