    Grade Change

    Only needed for students who are NA, NR, RM or WD or who have graduated. All other grade changes are done online. The Instructor/Department will complete this form and submit it to the Falk College Student Success Graduate Recorder. The Graduate Recorder will submit this form to the Registrar’s Office.

    Grade Option

    For registering a course as Pass/Fail or as an Audit. Falk College Student Success also needs a copy of this form for student records, please submit a signed copy to 330 Barclay Hall.

    Incomplete Grade

    For requesting an incomplete grade. An incomplete grade will calculate as an ‘F’ until an actual grade is submitted.

    Independent Study Proposals

    The Falk College Office of Student Success will submit the completed form to the Registrar’s Office after all signatures have been obtained.

    Leave of Absence

    For leaving for a semester or withdrawing from the University. A completed Leave of Absence form should be sent to the Falk College Office of Student Success at 330 Barclay Hall addressed to “ATTN: Graduate Recorder”.

    Petition to Faculty – Transfer Credit

    For transferring credits to Syracuse University. The Falk College Office of Student Success will submit the completed form to the Registrar’s Office after all signatures have been obtained.

    Petition to Faculty – Flag Petition

    For flagging a class. The Falk College Office of Student Success will submit the completed form to the Registrar’s Office after all signatures have been obtained.

    Petition to Walk – Commencement

    For an early participation in Commencement. Petitions to walk for commencements should be sent to the Falk College Office of Student Success at 330 Barclay Hall addressed to “ATTN: Graduate Recorder”.
